At A1, “I have a sore throat” was enough. At B1, you need to be able to describe a complaint over time, ask for a verwijzing — referral, understand the uitslag — result, and figure out what your insurance covers. Dutch medicine is organised around the huisarts — and that's the first thing you need to understand.

Nora has been having trouble with her knee for a while. It's swollen and she has a nagging pain. She's worried about it.

At first the GP says: 'Let's wait and see for two weeks.' But it doesn't get better. Then he refers her to the orthopaedic outpatient clinic, with a referral.

At the hospital there's an examination: an ultrasound and a blood test. A week later the doctor calls with the result: it's an inflammation, nothing serious. The treatment: rest, ice and physiotherapy.

Then the hospital bill arrives: 340 euros. That comes off her deductible. The physiotherapy is covered by her supplementary insurance, though. 'I still feel rotten,' says Nora, 'but at least now I know what it is.'

In the evenings and at weekends, you don't call the ambulance, but the huisartsenpost: they decide by phone whether you need to come in. Usually, such a post is located at a hospital.
