Alex has had trouble with his shoulder for two weeks. The pain spreads to his arm, and at night he sleeps badly because of it. He calls the GP.

The assistant asks whether it's urgent. He can come tomorrow. The GP examines him and says: 'I'll give you a referral to the physiotherapist.'

But will it be covered? Physiotherapy isn't part of the basic insurance. Luckily Alex has supplementary insurance: nine treatments a year.

In May he already had to see a specialist at the hospital. He paid that bill himself, from his deductible. Luckily they get a healthcare allowance, because the premium isn't cheap either.

2. How the system works

Who is responsible for what
NederlandsWhat it is
de huisartsGP, first point of contact
de assistentedoctor's assistant: appointments, assesses urgency
de praktijkondersteuner (POH)specialist at the practice: diabetes, mental health
de specialisthospital doctor, by referral only
de verwijzing / doorverwijzingreferral to a specialist
de huisartsenpost (HAP)out-of-hours service in the evenings and at weekends
de spoedeisende hulp (SEH)emergency department
112ambulance, for life-threatening situations
de apotheekpharmacy, dispenses by prescription
de tandartsdentist, usually outside basic insurance

Without a verwijzing from your huisarts, a hospital specialist will not see you, and your insurance will not cover the consultation. This is the main difference from the system many are used to.

3. Insurance and money

De zorgverzekering
NederlandsWhat it means
de basisverzekeringmandatory basic insurance
de aanvullende verzekeringsupplementary (dentist, physio)
de premiemonthly premium
het eigen risicoexcess: first expenses paid by yourself
de zorgtoeslaggovernment allowance for insurance
de vergoedingreimbursement of expenses
valt onder de verzekeringcovered by insurance
de nota / de rekeningmedical bill
declarerenclaim reimbursement
overstappenchange insurer (once a year, by 31 December)

A consultation with a huisarts is not part of the eigen risico — it's free. However, tests, scans, and specialist visits are first deducted from your excess.
