Healthcare and welfare

Talking to a doctor at B1 is about symptoms. At B2, the system is added: who refers, who pays, what eigen bijdrage is, how ggz differs from huisartsenzorg. These words appear in letters and determine whether you get to see a specialist or wait six months.

Marjolein, the neighbour, has been caring for her father for two years. He has dementia and still lives at home. As his carer she's with him almost every day, alongside her job. She's overburdened.

Through the GP, district nursing is arranged: twice a day a nurse helps with washing and medication. That's covered by the basic insurance, without the deductible.

For other help she has to go to the municipality. A kitchen-table conversation follows: an adviser comes round and looks at what's needed. Her father gets help in the home through the Wmo, with a small personal contribution.

A nursing home requires an official needs assessment, and there's a long waiting list. To give Marjolein a break, her father now spends one weekend a month at a respite house: respite care. For her own stress she talks to the mental health practice nurse at the GP's.

Who is responsible for what

Three doors into the system
  • Who

    de huisarts

    What they are responsible for

    almost all primary care

    How to access

    book directly, same-day appointment

  • Who

    de specialist

    What they are responsible for

    hospital treatment

    How to access

only with a verwijzing from the huisarts

  • Who

    de ggz

    What they are responsible for

    mental health

    How to access

also by referral, often with a waiting list

  • Who

    de wijkverpleging

    What they are responsible for

    home care

    How to access

    via an indicatie

  • Who

    de gemeente (Wmo)

    What they are responsible for

    daily life: assistance, transport, adaptations

    How to access

    application and keukentafelgesprek

The huisarts is the gatekeeper of the system: without their referral, the insurance company usually won't pay for a specialist. Hence the importance of the phrase ik wil een verwijzing.

    Comment

If a clinic is niet-gecontracteerd, the bill is only partially covered — usually 70–80%. This question should be asked before the first appointment, not after.

A Keukentafelgesprek literally means “kitchen table conversation”: a municipal employee comes to your home to determine what assistance you are entitled to. The name is informal, but the consequences are quite official.

Gesprek — Een verwijzing vragen

Mila has been waiting a month for improvement and is asking for a referral.

  1. Mila

De klachten zijn niet minder geworden. Ik zou graag een verwijzing willen.

The symptoms haven't got any better. I'd like a referral.

  2. Dokter Visserhuisarts

Dat kan. Naar welke specialist denkt u zelf?

That's possible. Which specialist do you have in mind?

  3. Mila

Iets met sport en houding. Wordt fysiotherapie vergoed?

Something to do with sport and posture. Is physiotherapy covered?

  4. Dokter Visserhuisarts

Alleen uit uw aanvullende verzekering. De basis vergoedt de eerste twintig behandelingen niet.

Only from your supplementary insurance. The basic insurance doesn't cover the first twenty sessions.

  5. Mila

En het eigen risico? Gaat dat er nog vanaf?

And the compulsory excess? Does that still come off?

  6. Dokter Visserhuisarts

Bij de specialist wel, bij mij niet. Huisartsenzorg valt er buiten.

At the specialist, yes; with me, no. GP care doesn't count towards it.

  7. Mila

Hoe lang is de wachtlijst ongeveer?

Roughly how long is the waiting list?

  8. Dokter Visserhuisarts

Zes tot acht weken. Ik zet erbij dat het urgent is, dat scheelt soms.

Six to eight weeks. I'll mark it as urgent; sometimes that helps.
